You have to go to the NPC at lith harbor to get the medal, and only the leader of the family can get it.

Leader can only get it. Btw thers another medal. Theres a quest that gives you. Sucks though.+1 everything

Not just the leader can get it. Anyone who has 1000 members under you, can get the medal. Go to the npc in lith.
