Does a chaos scroll 60% effect all the slots at once or just one at a time?

Yes, each successful or unsuccessful scroll will take up one (1) slot But tis' better to sell them ^.^

A chaos scroll takes each stat individually, and either increases or decreases it. So if you stat out with a staff with 4 INT and 80 MAtt, the chaos scroll may change it to a 6 INT 75 MAtt staff.

I think he's asking if a chaos scroll is like a normal scroll and just takes up 1 slot when used or if it takes up more than one slot. Assuming thats your question, when you use a chaos scroll, only one slot is taken up.

Takes the use of one slot per scroll. It can affect every single stat on the equip, anywhere from -5 to +5, including 0 (so it may not do anything). That is, if it even succeeds in the first place.

Erm... I'm pretty sure he meant stats. Ex: 2 ATK, 7 DEF PAC + Chaos Scroll = 7 ATK, 2 DEF PAC. (Pssh, I wish...)

lolz it is basically a normal scroll uses 1 slot but worth it^^ if ur lucky u can make like the godliest items ever such as a 32luk zhelm>which i made and c/o was 6b(2b meso+4b worth in item) but i sold for $500us cash lolz
