to get that medal u have to get S rank in 5 Diff pq's but, wut if u failed for many times? that means you can't get it? or there's a ratio so u can do more success to overcome the failure then u get S rank? and what is the stats on it btw?

3/3/3/3 and some gimmick stats. If you failed many times, it just means you have to do more to get that percentage up. So, like LPQ; if you have 89/100, you wont S rank it, but a 99/110, it will be a S rank.

Yes, you can increase rank by increasing the number of completions if you don't have a 90% completion rate (KPQ, LPQ, etc) at 100 runthroughs. Other PQs, such as CPQ, only require a 70% completion rate; Ariant PQ requires a 40% completion rate -- however, for those, you need to get first place as well as complete it 100 times.
