Because he quit right after he had reached lvl 200 and the npc thingies were out after he had quit.

You have to do a quest for it. He did not do the quest.

Because, he got to level 200 before that was out. He never bothered to do it afterwards. After he got 200, the only time he pretty much logged in was the 3rd Anniversary to fight the cake monster. Now, never/hardly ever.
