I think there should be restrooms, so your little avatar can go when nature calls. And have a little bar that fills up, and if it goes over the max limit, you die!

LOL! We could have a BP bar (bladder points). Whenever you take potions, your BP bar fills up. The amount of BP compared to your MaxBP determines how much HP you lose every minute. Also, equips can increase MaxBP.
